Below you will find various forms and documents you need to establish and administer your donor advised fund account with The Fund For Philanthropy.

 

To submit a form, please download a copy to your computer, complete and save it, and then email, mail, or fax it to TFFP. If sent electronically, your type written name is accepted as your signature. The completed, saved form can be emailed to TFFP at info@fundforphilanthropy.org. Alternatively, you may print the form, complete it by hand, and mail or fax the form to TFFP. TFFP Fund Application (Fillable online PDF)

This is Step #2 for opening a donor-advised fund account with TFFP. With this form we collect pertinent donor/advisor information to help manage your account.

 

TFFP Contribution Form (Fillable online PDF)

Once your account is open, you can make contributions to your TFFP account. If you prefer to print, complete, and mail or fax the form, or if you want to make a contribution from a securities account, you can use this form.

 

TFFP Investment Option Form (Fillable online PDF)

You can allocate (or reallocate, once a quarter) your investment strategy for your account with this form. If you prefer to print, complete, and mail or fax your allocation, or make other changes to your investment options, you can use this form.

 

TFFP Grant Recommendation Form (Fillable online PDF)

You can always recommend a grant with this form. If you prefer to print, complete, and mail or fax a grant recommendation, you can use this form.

Instructions for Viewing, Downloading and Printing a PDF Document

 

To view, download, or print a PDF document, you must have Adobe Acrobat Reader version 3.01 or later installed on your computer. To print a PDF document, you need a printer with graphics capabilities. Instructions for obtaining the free Acrobat Reader software are available on the Adobe Acrobat Reader site.

 

How to use Adobe Acrobat Reader

 

To view a PDF file in your web browser’s window, click on the links to the PDF file above. Depending on how your browser is configured, you may not be able to view the PDF file right away when you click on the link. If you get a dialog box:

 

  • Click on the “Save” option.
  • Designate where on your hard drive you want to save the PDF file and note the file name.
  • To open the file after you’ve saved it, locate the PDF file in your computer’s directory and double-click on it.

 

To save a PDF file to your computer:

 

  • Click on the link to the PDF file using your right mouse button (for Windows systems) or click and hold (for Macintosh systems).
  • Choose the “Save As,” “Save Target As,” or “Save Link As” menu item from the drop-down menu.
  • Designate where on your hard drive you want to save the PDF file and note the file name.
  • To open the file after you’ve saved it, locate the PDF file in your computer’s directory and double-click on it.
  • To print a PDF file, click on the Printer icon on the Acrobat Reader toolbar while the file is open.
